A 74-year-old man with hypertension and a 60-pack-year smoking history is brought to the ED for sudden severe abdominal and back pain. He is diaphoretic and confused. Blood pressure is 78/46 mm Hg, pulse is 124/min, and respirations are 24/min. The abdomen is tender with a pulsatile periumbilical mass. Two large-bore IVs are placed and blood is sent for type and crossmatch. Which diagnostic test is the best initial choice?
Ruptured abdominal aortic aneurysm causes retroperitoneal hemorrhage with shock; the bedside test that rapidly confirms aneurysm and changes management is abdominal ultrasonography. B is tempting because CT angiography defines anatomy, but contrast CT is for hemodynamically stable patients and delays operative consultation. C is correct: it is immediate, noninvasive, and appropriate while resuscitation and vascular surgery consultation proceed. A is too slow and inappropriate in shock. A may screen stable outpatients but is not an ED test for suspected rupture.

Learners
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
