A 2-year-old girl is brought to the emergency department for 1 day of vomiting and fever. She is sleepy but arousable. Temperature is 39.1°C, heart rate is 158/min, respiratory rate is 36/min, blood pressure is 90/56 mm Hg, and oxygen saturation is 99% on room air. Capillary refill is 3 seconds, and mucous membranes are dry. Which interpretation of these vital signs is most appropriate?
Toddlers normally have higher heart and respiratory rates than adults, but HR 158/min and RR 36/min with fever, delayed capillary refill, and dry mucosa suggest compensated dehydration or early sepsis; children often maintain blood pressure until late shock. D is incorrect because these findings are not simply normal age variation. C is correct: assess perfusion, hydration, mental status, and escalate for supervised fluid resuscitation and sepsis evaluation as indicated. B is incorrect because normal pulse oximetry does not exclude shock or dehydration. A is incorrect because adult blood pressure norms should not be applied; hypotension is a late pediatric sign.

Learners
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
