A 68-year-old woman is brought to the emergency department 40 minutes after sudden right arm weakness and expressive aphasia. She has atrial fibrillation and hypertension. Blood pressure is 176/94 mm Hg, pulse is irregular, and oxygen saturation is 97% on room air. Fingerstick glucose is 104 mg/dL. She is awake and following simple commands. Neurologic examination shows right lower facial droop, right pronator drift, and nonfluent speech.
Sudden aphasia and unilateral weakness localize to the left MCA territory and require emergency stroke evaluation. B is correct: first exclude hemorrhage with noncontrast CT while activating supervised stroke care. D is tempting for ischemia, but aspirin follows hemorrhage exclusion and thrombolysis assessment. C may help secondary prevention, but source workup must not delay acute imaging. A is unsafe because hyperacute deficits are time-sensitive even if transient improvement occurs.

Learners
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
