A 60-year-old woman comes to clinic for preventive care. She has rheumatoid arthritis treated with methotrexate and adalimumab. She had chickenpox as a child and has never received a zoster vaccine. She feels well today and has no fever. Her last influenza and COVID-19 vaccines were 1 year ago. Which immunization recommendation is most appropriate?
Immunosuppression increases varicella-zoster reactivation risk; recombinant zoster vaccine is nonlive and recommended for immunocompromised adults aged 19 years or older, including this 60-year-old. C is incorrect because live zoster vaccine is contraindicated with significant immunosuppression and is no longer used in the United States. A is tempting because vaccine response may improve off therapy, but routine permanent deferral leaves preventable risk. B is incorrect because varicella vaccine is live and used for varicella-naive patients, not zoster prevention in an immunosuppressed adult with prior chickenpox. Diagnose increased zoster risk; order recombinant zoster vaccine; treat with 2 doses; escalate if active disseminated zoster or severe immunocompromise questions arise.

The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
