A 28-year-old woman presents to an OMM teaching clinic with acute low back pain after a motor vehicle collision 2 hours ago. She reports new left leg weakness. Examination shows midline lumbar tenderness, guarded motion, and decreased left ankle dorsiflexion strength. She asks for lumbar HVLA because “the body can heal itself.” Which response best reflects osteopathic tenets and trainee scope?
Osteopathic tenets and safety
Osteopathic rational treatment integrates body unity, structure-function, and self-regulation, but red flags override manipulation. Acute trauma plus midline tenderness and new weakness suggests possible fracture, instability, or neurologic compromise; defer OMT and escalate. B is unsafe because HVLA is contraindicated when instability or neurologic deficit is suspected. A recognizes body unity but still bypasses needed evaluation. A overapplies self-healing and ignores neurologic deficit.
Diagnose red flags; order/seek urgent imaging or evaluation under supervision; treat with no spinal thrust; refer/escalate for trauma with weakness.
