You are the AEMT for a 67-year-old with crushing chest pressure, dizziness, and confusion. Skin is cool and pale. Vital signs: HR 38/min regular, BP 76/44 mm Hg, RR 20/min, SpO2 95% on nasal cannula. Your protocol allows oxygen, IV access, fluid bolus for hypotension, and escalation for care beyond AEMT scope; transcutaneous pacing is ALS-only locally. What is the best next step?
B is correct: hypotension and confusion indicate poor perfusion from symptomatic bradycardia; AEMT care should support circulation, follow protocol, request ALS, and transport. C is incorrect because nitroglycerin can worsen hypotension. D is tempting because pacing is appropriate for unstable bradycardia, but it is beyond local AEMT scope here, so ALS escalation is required. A is incorrect because orthostatic vitals and prolonged history delay care for an unstable patient. Review AEMT Scope Boundaries — focus on protocol-authorized interventions, ALS referral, and documentation of vital signs, treatments, response, and handoff.

The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
