A 64-year-old man with COPD is admitted for acute shortness of breath. The nurse calls respiratory care because he “looks worse,” but no current SpO2, respiratory rate, breath sounds, or mental status are documented. He is sitting upright and speaking in short phrases. In this phase of the simulation, which action is most appropriate first?
Dyspnea in COPD may reflect bronchospasm, hypoxemia, secretion retention, or ventilatory failure; the first simulation task is focused data gathering unless arrest is evident. C is correct because airway, work of breathing, breath sounds, SpO2, and vital signs identify the immediate respiratory problem. B assumes bronchospasm without confirming wheeze. D escalates before evidence of failed ventilation or airway loss. A targets secretions without signs such as coarse crackles or retained sputum.

The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
