A 64-year-old man has 45 minutes of crushing substernal chest pain radiating to the left arm. He is diaphoretic; ECG shows ST-segment elevation in leads II, III, and aVF. Serum troponin I is elevated. The final line of the item asks, “Which of the following biochemical events accounts for this patient’s elevated serum troponin?” What task type is this item primarily asking you to recognize?
D is correct: the diagnosis is acute inferior STEMI, but the question asks why troponin is elevated—myocyte necrosis with membrane disruption and protein release. B is tempting because the stem strongly signals MI, but the final sentence does not ask for the diagnosis. A is incorrect because ECG and troponin are already provided. C is incorrect because no answer choice is being sought for reperfusion or stabilization. Review Task identification in a vignette — focus on the final sentence and the requested decision rule.

Learners
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
