A 68-year-old adult is 10 hours postoperative after abdominal surgery. Over 2 hours, respirations increased from 18 to 30/min, oxygen saturation decreased from 96% to 89% on 2 L/min nasal cannula, and the patient is increasingly restless and using accessory muscles. Incisional pain is 6/10. Which nursing action has the highest priority?
A: Pain control is appropriate and may improve breathing, but ABCs require immediate respiratory assessment and support first. C: Correct. Worsening hypoxemia, tachypnea, restlessness, and accessory muscle use indicate an unstable breathing problem; the RN should assess and intervene immediately within protocol, then escalate if unresolved. D: Vital signs can be delegated, but interpretation and response to deterioration remain RN responsibilities. B: Incentive spirometry is important postoperative care, but it is not the first action when oxygenation is acutely worsening.

The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
