A 27-year-old man is brought to the emergency department by his roommate for 3 days of insomnia and new paranoia. He says cameras are hidden in the vents and gives a disorganized, inconsistent timeline. He denies substance use, but the roommate reports heavy cannabis use and recent job loss. Vital signs are normal; urine drug screen is pending. Which psychiatric history element is most important to obtain next?
Acute psychosis with an unreliable historian requires collateral history to define onset, substance relationship, functional decline, and danger to self or others. B is correct because collateral can distinguish primary psychosis from substance-induced symptoms and guide urgent disposition. A may help later but does not address immediate diagnostic reliability or safety. D is relevant for risk but is less urgent than the current timeline and substance exposure. C may suggest personality pathology, but acute paranoia and disorganization require safety-focused assessment first.

The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
