A 62-year-old man reports 3 months of substernal chest pressure when climbing two flights of stairs. The pain resolves within 5 minutes of rest and has not increased in frequency or severity. He has hypertension, hyperlipidemia, and a 30-pack-year smoking history. Resting ECG is normal, and he is pain-free in clinic. Which diagnosis best explains this pattern?
Fixed atherosclerotic coronary stenosis limits flow during exertion, causing predictable pressure relieved by rest or nitroglycerin: stable angina. A is tempting because it is ischemic pain, but unstable angina is new, crescendo, or occurs at rest. A results from coronary vasospasm, classically rest or nocturnal pain with transient ST elevation. A can cause exertional angina, often with nonobstructive coronaries, but the classic risk factors and reproducible exertional threshold favor fixed obstructive disease.

The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
