A 67-year-old woman presents to triage with sudden chest pain and dyspnea. She is diaphoretic and confused. Vital signs are BP 82/48 mm Hg, HR 132/min, RR 30/min, temperature 36.8°C, and SpO2 86% on room air. Her lips are cyanotic, and her hands are cool and clammy. As the PA student in the emergency department, what is the best immediate action?
Hypotension, tachycardia, hypoxia, cyanosis, cool extremities, and confusion indicate impaired oxygenation/perfusion with possible shock, so A is correct. B is tempting because chest pain needs ECG/troponin, but registration and routine placement delay stabilization. D is tempting for PE, but unstable patients should not leave triage for imaging before resuscitation. C is inappropriate because this is not stable isolated bronchospasm.

Learners
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
