A 72-year-old patient on a telemetry unit received morning carvedilol and IV furosemide 2 hours ago. Current BP is 88/54 mm Hg, down from 128/76 mm Hg; heart rate is 112/min. The patient reports dizziness when standing to walk to the bathroom. Skin is cool, and urine output has been 20 mL in the past hour. What is the priority nursing action?
D is correct. Circulation and safety are priority: symptomatic hypotension with tachycardia, cool skin, and low urine output requires immediate reassessment, fall prevention, perfusion assessment, and escalation. B is unsafe because additional antihypertensive therapy could worsen hypoperfusion. C is tempting for mobility needs, but ambulation is unsafe and assessment of an unstable cardiovascular change remains RN responsibility. A ignores a concerning trend; symptoms matter more than a single “borderline” number.

Learners
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
