During predeparture assessment for an interfacility ground transport, a 64-year-old patient with pneumonia is receiving oxygen by nonrebreather. The patient becomes restless. Vital signs change from SpO2 94% to 86%, respiratory rate 32/min, heart rate 118/min, and blood pressure 92/54 mm Hg. Breath sounds are diminished on the right. Packaging is not complete. What is the transport nurse’s priority action?
ABCs make airway and breathing the priority before packaging or departure. D is tempting because hypotension suggests circulation compromise, but oxygenation is deteriorating first. C is correct: assess airway, equipment, positioning, breath sounds, and support ventilation within protocol before transport. A is necessary for safety, but securing a hypoxic patient without addressing breathing delays treatment of an immediate life threat. A supports communication, but it comes after stabilizing the primary survey threat or escalating for help.

The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
