A 24-year-old man is brought to the trauma bay after a high-speed motor vehicle collision. He is obtunded with gurgling respirations. Temperature is 36.8°C, blood pressure is 118/76 mm Hg, pulse is 122/min, respirations are 8/min, and oxygen saturation is 84% on a nonrebreather mask. GCS score is 7. Breath sounds are equal bilaterally. He has facial abrasions and diffuse abdominal tenderness. Which next step should you recommend?
ABCs Before Imaging
A low GCS with gurgling respirations and hypoxemia indicates airway failure; trauma care prioritizes airway with cervical spine protection before diagnostic refinement. A is tempting for head injury, but CT follows stabilization. B is useful for hemorrhage evaluation, but breathing/airway comes first. A may be needed for hollow viscus injury or operative abdomen, but antibiotics/laparotomy do not correct immediate hypoxemia.
Clinician Action: Diagnose airway compromise by GCS ≤8, poor respirations, or hypoxemia; order adjunct trauma studies after stabilization; treat with protected intubation; escalate to trauma/anesthesia/surgery immediately.
