A 3-day-old term male newborn is brought for yellowing of the face and chest. He was born vaginally at 39 weeks, is breastfeeding every 2–3 hours, and has normal urine and stool output. Birth weight was 3.4 kg; current weight is 3.25 kg. He is alert, afebrile, and has no hepatosplenomegaly. Total bilirubin is 9.8 mg/dL with direct bilirubin 0.3 mg/dL. Which jaundice pattern is most likely?
Physiologic jaundice is transient unconjugated hyperbilirubinemia from high neonatal RBC turnover and immature hepatic conjugation; onset after 24 hours, peak days 3–5 in term infants, and low direct bilirubin fit D. C is tempting early, but breastfeeding jaundice has poor intake, excess weight loss, and scant stools. A begins after day 5–7 and peaks around week 2 despite good intake. A causes direct hyperbilirubinemia, pale stools, and persistent jaundice. Treat with feeding support and bilirubin risk stratification; phototherapy only if thresholds are met.

Learners
What MedExPrep learners say about the platform (reviews across all exams, not only COMAT Peds).
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
