A 68-year-old adult with atrial fibrillation is admitted for pneumonia. Home medications include warfarin 5 mg by mouth nightly, amiodarone 200 mg by mouth daily, and lisinopril 20 mg by mouth daily. The admitting team starts trimethoprim-sulfamethoxazole DS 1 tablet by mouth twice daily. As the pharmacist reviewing medication problems, which issue should be prioritized first?
D is correct. TMP-SMX inhibits warfarin metabolism and can cause a rapid INR rise; amiodarone already potentiates warfarin, making this high-priority bleeding risk. This is the most urgent medication problem because it is safety-critical and preventable.
NOTICE: warfarin plus amiodarone plus TMP-SMX. SAY: recommend an alternative antibiotic or empiric warfarin dose reduction with prompt INR follow-up. MONITOR: INR within 2-3 days and bleeding daily. HOLD/CLARIFY/ESCALATE: escalate immediately for active bleeding or supratherapeutic INR.
B: tempting because illness can affect ACE inhibitor tolerability, but cough is not the urgent issue; the immediate high-risk interaction is warfarin-TMP-SMX. A: tempting because infection changes therapy, but oral amiodarone does not require IV conversion here. C: tempting if stable outpatient monitoring is assumed, but delayed INR follow-up is unsafe after starting TMP-SMX.

The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
