A master's-level social worker at a community mental health agency meets a new adult client who says, “I need help, but I’m not sure what kind.” The client reports job loss, poor sleep, and conflict with a partner, but denies current thoughts of self-harm or harm to others. The intake form is incomplete. The question asks for the initial action before developing a service plan. What should the social worker do first?
A is correct because the stem asks for the initial action before planning; assessment clarifies the presenting problems, risks, strengths, and environment. C is premature intervention without sufficient assessment. A may be appropriate later if symptoms suggest medication evaluation, but prescribing decisions are outside the social worker’s role and referral is not the first step here. A skips assessment and evaluation cannot occur before goals and interventions exist.
