What is the best next action?
Patient identification and order verification
D is correct: two identifiers plus order matching help prevent wrong-patient or wrong-procedure CT. The discrepancy in exam type, contrast use, indication, or body part must be resolved before exposure. A uses only one identifier and ignores conflicting information. B improperly changes the provider order/protocol without authorized clarification. C exposes the patient before confirming the correct examination.
Clinician Action: Verify identity, order, body part/laterality, indication, and contrast plan; stop and escalate discrepancies per department policy.
