A 62-year-old man in inpatient rehabilitation after a right middle cerebral artery stroke has difficulty clearing his left foot. During walking, the left leg remains relatively extended, the pelvis hikes, and the foot swings outward in a semicircular path during swing phase. Left ankle dorsiflexion is mildly weak, but tone is increased in plantar flexors and knee extensors. Which gait mechanism best explains this pattern?
Upper motor neuron extensor synergy causes stiff knee extension and plantar flexion, so the limb clears by hip hiking and circumduction; treat with stretching, tone management, ankle-foot orthosis, and task-specific gait training. B is tempting because pain alters stance, but antalgic gait shortens painful stance time, not swing arc. A causes pelvic drop on the opposite side during stance. A causes high-stepping hip and knee flexion from flaccid dorsiflexor weakness, not increased tone.

The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
