A 54-year-old patient has acute right orbital pain, complete ptosis, limited adduction/elevation/depression, impaired abduction, and decreased forehead sensation. Visual acuity, color vision, pupils, and fundus examination are normal, with no relative afferent pupillary defect. MRI shows a small enhancing lesion involving the posterior orbit. Which anatomic compartment best localizes this lesion?
B is correct: cranial nerves III, IV, VI, V1, and sympathetics traverse the superior orbital fissure, while the optic nerve is spared. C is tempting because it is posterior, but the optic canal primarily contains CN II and the ophthalmic artery. A is close, but orbital apex lesions usually add optic neuropathy. D can cause similar ophthalmoplegia and V1 findings, but the stem localizes to posterior orbit without cavernous sinus features.

Learners
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
