DOMAIN: Neurocritical Care → Objective: prevent secondary brain injury → Subskill: airway, hemodynamics, sedation. A 24-year-old man is brought after a high-speed collision. He opens eyes to pain, makes incomprehensible sounds, and flexes to pain. Pupils are equal and reactive. CT is pending. Oxygen saturation is 90% on nonrebreather; blood pressure is 108/62 mm Hg. Cervical injury is possible. Which immediate management best protects against secondary brain injury?
Severe traumatic brain injury with GCS 7 and hypoxemia requires airway protection; rapid-sequence intubation should avoid hypoxia and hypotension. For this 24-year-old, systolic pressure should be maintained at least 110 mm Hg. B is tempting because sedation preserves imaging workflow, but GCS ≤8 with hypoxemia mandates intubation. D is correct: preoxygenation, inline stabilization, hemodynamic support, and exam-aware sedation reduce secondary injury. C is reserved for acute herniation; prophylactic severe hyperventilation risks cerebral ischemia. A worsens outcome because hypotension is strongly associated with mortality; osmotherapy does not precede airway rescue in an unstable hypoxemic patient.

Learners
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
