A 68-year-old woman with metastatic pancreatic cancer is receiving second-line chemotherapy. She has worsening abdominal pain, nausea, weight loss, and two hospitalizations in 6 weeks. She values more time at home but wants to continue cancer treatment if it may help. Her daughter asks whether palliative care means “giving up.” Which response is most appropriate?
Palliative care is need-based care for serious illness: it treats symptoms, supports communication, and helps align treatments with goals at any disease stage. C is correct because refractory symptoms, repeated hospitalizations, and complex goals warrant concurrent palliative and oncologic care. B confuses palliative care with hospice; hospice usually emphasizes comfort when disease-directed therapy is no longer goal-concordant. A is incorrect because code status is not a prerequisite for palliative consultation. D is incorrect because referral should not wait for a specific prognosis when symptom and communication needs are present.

Learners
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
