1.1.1.3 Define the complaint before you solve it

A patient saying “I need a crown,” “I want this tooth pulled,” or “my filling fell out” is giving you a starting point, not a final plan. The single rule that resolves the conflict is this: the chief complaint identifies the task; it does not authorize the procedure. In DLOSCE-style reasoning, you must translate the patient’s words into a clinical target: pain source, swelling source, trauma pattern, esthetic concern, functional issue, or preventive request.

Start with an open-ended invitation: “Tell me what brought you in today.” Then narrow the history with focused questions about onset, location, quality, severity, duration, triggers, progression, associated swelling, fever, bad taste, trauma, and impact on chewing or sleep. If the complaint is nonpainful, ask what change the patient noticed, how long it has been present, and whether it bleeds, grows, or interferes with function. This preserves diagnostic neutrality and avoids anchoring too early.

Urgent complaints include uncontrolled pain, acute fracture with sharp edges or exposed tissue, new swelling, posttreatment complications, trauma, and bleeding. Mixed visits are especially testable.
